Voice training, surgery, or testosterone?

Recently, while I was on TikTok Live, I had a great conversation with a singer who has been working on masculinizing their voice. They shared with me their uncertainty about the best path forward—whether to focus on voice training, take testosterone (T), or consider masculinizing surgery. This is a question I encounter often, and it’s a deeply personal and challenging decision that doesn’t have a one-size-fits-all answer.

The Dilemma: Training, Testosterone, or Surgery?

During our conversation, the singer asked a thought-provoking question: “Is it likely to get a decently masculine voice through training, similar to voice masculinization surgery? I want a lower voice for gender affirmation and singing, but if I go on T, I might lose those high notes.”

The first thing I often recommend, which I also mentioned during the live, is to start with voice training. Almost any surgeon will advise you to train first because it provides you with a foundation. While training may not give you the exact results as surgery, it will still produce significant changes—and perhaps those results will align with what you’re hoping to achieve.

Weighing the Risks: Surgery and Testosterone

Voice masculinization surgery and taking testosterone both carry risks, especially when it comes to preserving your singing range. With surgery, for instance, you might lose some of your high notes, depending on the type of procedure. That’s why I always emphasize the importance of doing thorough research on the specific surgery you’re considering. Some surgeries may have even more dramatic risks than taking T, and it’s crucial to understand these risks before making any decisions. But remember that you're choosing to take testosterone or have surgery because the benefits outweigh the risks!

Resources for Singers on Testosterone

For those who do decide to take testosterone, there are excellent resources available for singers. One resource I highly recommend is Eli Conley, who runs masterclasses specifically designed for people on T. These classes can provide invaluable guidance on retraining your high notes after the changes that come with testosterone.

Making the Right Choice for You

In the end, the choice between voice training, testosterone, or surgery is deeply personal, and only you can make the right decision for yourself. Each path has its benefits and risks, and it’s essential to weigh these carefully in the context of your goals for gender affirmation and singing. Remember, your journey is uniquely yours, and whatever path you choose, there are tools and resources to support you along the way.
